Tear troughs, hollows, and the shadows that read as fatigue

The under-eye is one of the most expression-rich areas on the face, and one of the thinnest in terms of skin and tissue support. The tear trough is the groove that runs from the inner corner of the eye outward, sitting along the orbital rim. As volume is lost from this area, light scatters differently. Shadow appears. The face reads tired even when energy is high.

Treating the tear trough requires anatomic precision. The infraorbital nerve sits nearby. The blood vessels are surface-close. Hyaluronic acid placed even slightly too superficial can cause the Tyndall effect, a blue-gray discoloration that lasts as long as the product does. This is advanced injector territory.

How under-eye filler restores the tear trough

The under-eye has three layers that contribute to its appearance.

Skin here is among the thinnest on the body. It's prone to fine lines, shadowing, and showing the vasculature beneath.

Fat compartments below the eye lose volume with age, deflate, and shift forward, creating the hollow-with-puffiness pattern many clients describe.

Orbital bone beneath everything else changes shape across decades, with the orbital rim resorbing and the eye socket appearing larger.

Treatment that addresses only one of these layers often fails. The most effective plans consider all three.

How this changes over time

Done well, no one can tell anything was done. Done poorly, everyone can.

Under-eye changes appear earlier than most facial changes, often in the early thirties. The skin thins, the fat compartments shift, and the orbital rim begins to recede slightly. Clients describe looking tired even when rested.

Sleep, hydration, allergies, and genetics all play roles. So does the camera angle of how often you see yourself. Selfies show the under-eye in unflattering light most of the day.

In Your 20s

Most under-eye concerns in this decade are genetic, not age-related. Some clients have always had hollowing or shadowing from anatomy. Treatment in this decade is rare and reserved for clear structural issues.

In Your 30s

Subtle hollowing begins as fat compartments shift and the orbital rim starts to change. Clients first notice they look tired in photos despite sleeping well. This is when most under-eye treatment plans begin.

In Your 40s

Volume loss continues, the orbital rim recedes more visibly, and skin quality begins to play a larger role. Treatment often combines tear trough filler with surface work like Skinvive or gentle laser.

In Your 50s+

The most successful plans address all three layers: structural filler where appropriate, surface treatments for skin quality, and honest conversations about when surgical referral is the right move. Volume restoration becomes about the entire midface, not just the under-eye.

How we treat the under-eye

The under-eye is one of the most demanding injectable areas. We approach it with extreme conservatism.

We use needle technique placed deep, against the periosteum, with minimal product. The HA fillers of choice are Juvederm Volbella and RHA 2, which are soft, low-water-binding, and integrate smoothly with the thin tissue here. We never use thicker fillers in this area.

Dosing is small. Most first treatments use less than half a syringe across both sides. We'd rather underfill and revisit in two weeks than overshoot in one visit.

For some clients, the under-eye isn't the right place to treat. The cheek often deflates first, dropping the support beneath the under-eye and creating the shadow. Treating the cheek can correct the under-eye indirectly with less risk. We'll tell you which approach fits your face.
