What it is

The under-eye is one of the most expression-rich areas on the face and one of the thinnest in terms of tissue support. The tear trough is the groove that runs from the inner corner of the eye outward along the orbital rim. As volume is lost from this area, light scatters differently. Shadow appears. The face reads tired even when energy is high.

Some clients have genetic under-eye hollowing from anatomy. Others develop it gradually with age. The treatment plan depends on which.

Clients come in because they look tired in photos and concealer doesn't fully fix it. They've developed shadows that catch light at certain angles, particularly under harsh overhead lighting or selfie cameras. They want a refreshed look but are wary of over-filled under-eyes they've seen on others. They want the result no one names.

What Causes It

Three layers contribute to the under-eye's appearance.

Skin thinness in this area is among the most extreme on the body. It shows the vasculature beneath, fine lines, and shadowing readily.

Fat compartment changes beneath the eye cause volume to shift and deflate over time, creating the hollow-with-puffiness pattern clients describe.

Orbital bone changes across decades cause the rim to resorb, making the eye socket appear larger and the under-eye more sunken.

Common Signs

The signs depend on cause. Clients with genetic hollowing have had shadows their whole life. They notice them in selfies and certain lighting.

Clients with age-related changes describe looking tired in photos even after a full night's sleep. The concealer routine takes longer. Selfies show shadow that wasn't there a decade ago.

Why It Changes Over Time

Under-eye changes appear earlier than most facial changes, often in the early thirties. The skin thins, fat compartments shift, and the orbital rim begins to recede.

Sleep, hydration, allergies, and genetics all play roles in how visible the changes become. Selfies show the under-eye in unflattering light most of the day, which is why the area often becomes a primary concern.

How It's Commonly Addressed

The industry treats under-eye hollows with HA filler placed in the tear trough. Volbella, RHA 2, and Restylane are common choices for their softness and low water-binding.

The industry problem is over-treatment and wrong-product selection. Too much filler creates puffy under-eyes that look worse than the original hollow. Wrong products in the wrong layer cause the Tyndall effect, a blue-gray discoloration that lasts as long as the product does.

How we approach the under-eye

The under-eye is one of the most demanding injectable areas. We approach it with extreme conservatism.

We use needle technique placed deep, against the periosteum, with minimal product. Juvederm Volbella and RHA 2 are our HA filler choices for their softness, low water binding, and smooth integration with the thin tissue here. We never use thicker fillers in this area.

Dosing is small. Most first treatments use less than half a syringe across both sides. We'd rather underfill and revisit in two weeks than overshoot in one visit.

For some clients, the under-eye isn't the right place to treat. The cheek often deflates first, dropping the support beneath the under-eye and creating the shadow. Treating the cheek can correct the under-eye indirectly with less risk. We'll tell you which approach fits your face.
