What it is

Oily skin is characterized by elevated sebum production from the sebaceous glands. It's largely genetic but influenced by hormones, stress, climate, and diet.

The benefits of oily skin are real. It tends to age more slowly than dry skin. Fine lines develop later. The skin barrier is naturally protective. The downsides are visible shine, frequent congestion, and tendency toward breakouts.

Management aims for balance rather than elimination. The goal is to reduce excess shine and congestion without disrupting the natural oil production that protects the skin.

What Causes It

Several factors drive sebum production.

Genetics are the primary determinant of baseline sebum production.

Hormones particularly androgens, drive sebaceous gland activity. Hormonal fluctuations (puberty, menstrual cycle, pregnancy, perimenopause, PCOS) can shift oil production significantly.

Climate and humidity affect perceived oiliness. Hot, humid weather increases visible shine.

Skincare products can either help or worsen the appearance. Over-cleansing or stripping products often triggers compensatory increased oil production.

Common Signs

Oily skin shows shine across the T-zone, particularly on the forehead, nose, and chin. Makeup wears off quickly. Pores may appear enlarged. Frequent breakouts and congestion are common.

The skin often feels healthy and resilient with appropriate care, despite the visible oiliness.

Why It Changes Over Time

Oily skin tends to be most prominent during adolescence and the twenties, gradually decreasing through the thirties and forties. By the fifties, many clients with previously oily skin have transitioned to a more balanced or even dry skin type.

Hormonal shifts (pregnancy, perimenopause) can temporarily change the pattern. Most clients see their skin gradually balance over decades.

How It's Commonly Addressed

Oily skin management combines several modalities.

HydraFacial with targeted boosters for oily skin clears congestion and balances surface oil.

Chemical peels with salicylic acid penetrate sebum and clear follicles.

Retinoids regulate sebum production over time when used consistently.

Niacinamide topicals reduce oil production and improve barrier function.

Botox microdosing (intradermal Botox in the face) is an emerging treatment that can reduce oil production. We discuss it case-by-case.

How we approach oily skin

Our approach respects the natural protective function of oily skin while addressing the appearance concerns.

For most clients, the foundation is gentle but consistent skincare. We prescribe medical-grade retinoids (tretinoin or adapalene), niacinamide serums, and oil-balancing cleansers without harsh stripping ingredients.

In-office treatments include HydraFacial with oily-skin boosters, salicylic acid peels for deeper congestion, and gentle exfoliation. Treatments every four to six weeks maintain balance.

For clients with persistent visible shine, we may discuss Botox microdosing as an option. It's not a first-line treatment, but for the right candidate it can reduce oil production meaningfully.
