What it is

Lip asymmetry is the visible imbalance between sides of the lip, between upper and lower lip, or between specific structural points (cupid's bow peaks, mouth corners).

Some asymmetry is normal and reads as character. The lip work clients seek is for asymmetry that reads as uneven or distracting, not for the small asymmetries that make a face look like a person.

Why Patients Seek Treatment

Most clients come in having noticed the asymmetry in photos. They've often always known their lips were slightly uneven but recently it's become bothersome. Some clients seek treatment after previous filler work made the asymmetry more visible.

What Causes It

Lip asymmetry has several origins.

Genetic anatomy is the most common. Most faces are slightly asymmetric, and lips often reflect that.

Trauma or surgery can create asymmetry through scar tissue or altered structure.

Previous filler placement elsewhere can create or reveal asymmetry.

Neurological causes like Bell's palsy or other facial nerve issues can produce asymmetry that's not anatomic. These require medical assessment before aesthetic treatment.

Common Signs

The most common pattern is asymmetric cupid's bow (one peak higher than the other) or asymmetric lip body (one side fuller than the other).

Mouth corners may be uneven, with one side higher at rest. The smile may be uneven, though smile asymmetry is often a different concern requiring different treatment.

Why It Changes Over Time

Lip asymmetry typically stays consistent across decades unless trauma, surgery, or neurological factors intervene. Age-related volume loss can sometimes make existing asymmetry more visible as the lip thins around it.

How It's Commonly Addressed

The industry treats lip asymmetry with asymmetric filler placement, adding small amounts to the side or area that reads thinner. Some clinics treat aggressively, attempting full symmetry, which often makes asymmetry more obvious.

The key is recognizing that perfect symmetry usually isn't the goal. Studied asymmetry that reads as balanced is.

How we approach lip asymmetry

We map the lip you actually have before deciding where product goes. Most faces are slightly asymmetric, and forcing symmetric injection often makes the asymmetry more obvious, not less.

Treatment is precise and conservative. We may add small amounts to specific points, the cupid's bow peak, the lateral upper lip, the lower lip body, depending on what reads as uneven. We don't reflexively chase symmetry.

For asymmetry from neurological causes, we coordinate with the client's medical team before aesthetic treatment. For asymmetry from previous filler, we may dissolve and rebuild rather than add on top.
