Most clients don't realize how much the cheek changed until they see a side-by-side of a photo from a decade ago. The change is gradual. The effect is structural.

Cheek volume loss is the gradual deflation of the fat compartments that give the midface its convex shape. The malar fat pad, the deep medial fat pad, and the buccal fat pad each contribute. As they shrink and shift downward, the youthful triangle inverts: the cheek flattens, the lower face widens, and the entire structure of the face appears to drop.
The change is structural, not surface. Skincare doesn't address it. Filler does.
Clients come in for marionettes, nasolabial folds, or jowls and learn that the cheek is the answer. The cheek is the structural anchor that supports everything below it. Restoring it often improves multiple concerns indirectly without treating them directly.
Three factors drive cheek deflation.
Fat compartment atrophy is the primary cause. Each compartment thins gradually with age.
Bone resorption changes the underlying scaffold. The cheekbone itself recedes slightly with age, reducing the support for the soft tissue above.
Weight changes accelerate the process. Significant weight loss in a short period often produces visible cheek hollowing.
Most clients first notice cheek changes indirectly. They've started to look flatter or tired in the midface. The nasolabial folds have deepened. The jawline has softened.
The visible change in the cheek itself is often missed because it happens slowly. The downstream effects on the lower face are usually what brings clients in.
Cheek deflation begins in the thirties and accelerates through the forties and fifties. Each fat compartment loses volume at a different rate, which is why some clients deflate evenly and others develop asymmetry as they age.
Pregnancy, breastfeeding, weight changes, and significant exercise all affect the timeline. Genetics determine the baseline pattern.
The industry standard is HA filler placed in the cheek compartments. Voluma and Restylane Lyft are the most common choices. Some clinics use Sculptra for gradual collagen-stimulating restoration. Radiesse enters the conversation for specific structural lift.
The problem in the industry is over-treatment. The Instagram face comes from too much cheek filler. The result reads obvious in person and in photos.
