What it is

The masseter is the chewing muscle at the back of the jaw. When it's overdeveloped, often from teeth grinding, jaw clenching, or genetics, it bulks up. The lower face widens. The jawline reads square or boxy rather than soft.

Treatment is neuromodulator placed directly into the masseter to relax it. The muscle thins over months. The lower face softens.

Why Patients Seek Treatment

Clients come in for the aesthetic reason (softer jawline) or the functional reason (grinding, TMJ, headaches). The treatment is often the same. Once they hear neuromodulator can address both at once, many move forward.

What Causes It

Three factors drive masseter hypertrophy.

Teeth grinding or bruxism, often at night, is one of the most common causes. The masseter contracts repeatedly while you sleep.

Jaw clenching during stress or focus develops the muscle similarly to lifting weights.

Genetics determine baseline muscle bulk. Some clients have naturally larger masseters without grinding or clenching.

Common Signs

Clients with masseter hypertrophy often have a wide lower face that's disproportionate to their upper face. The jaw reads square or boxy.

Some clients have associated symptoms: morning jaw soreness, headaches, TMJ pain, or worn tooth enamel. The aesthetic concern and the functional symptoms often respond to the same treatment.

Why It Changes Over Time

Masseter hypertrophy can develop at any age but often becomes more pronounced through the twenties and thirties. Stress, jaw clenching habits, and untreated bruxism all contribute.

Without treatment, the muscle continues to develop. With consistent neuromodulator, it gradually thins over six to twelve months and the face softens noticeably.

How It's Commonly Addressed

The industry standard is Botox, Dysport, or Daxxify injected into the masseter muscle. Dosing varies widely between clinics, from twenty-five to fifty units per side.

The challenge is calibration. Too much can affect chewing strength. Too little doesn't soften the jaw or reduce grinding.

How we approach masseter treatment

We calibrate masseter treatment to the muscle in front of us. Smaller, less bulky masseters use less product. Larger, more developed masseters use more.

Most plans start with twenty-five to forty units of Botox per side, or the equivalent in Dysport or Daxxify. We assess at six weeks and may add more if needed.

Results show gradually over six to twelve weeks. The first treatment softens grinding symptoms quickly. The visible jaw softening takes longer because the muscle has to atrophy from disuse.
